Submission of Documents Prior to Arrival On Campus

Dear Student Athlete,

We are very excited that you will be attending Seminole State College and will be participating in intercollegiate athletics this year. In order for you to participate in any athletic activities, you will need to immediately complete and submit to the SSC Athletic Department the first four items listed on the checklist located on the side bar to the right of this letter.

Do not wait until you arrive on campus to turn in these items unless your have committed to SSC less than two weeks prior to your scheduled arrival on campus. Please read and fill out each document carefully. Be sure each document includes all of the required dated signatures. The completed documents can be mailed to the address below or they can be delivered in person. Do not return the Drug Testing Policy with your documents, keep it for your reference.

Students will not be permitted to participate in any physical activity until documentation of a physical exam has been submitted. Seminole State College does not provide physical exams or funds for physical exams for student athletes.

There are also a number of equally important items that require interaction with campus offices other than athletics, which need to be completed immediately. These items are shown in section II of the checklist. If you have questions regarding the completion of any of the items outside of athletics, contact the appropriate office directly or your coaching staff for assistance. There are also a small number of tasks that need to be completed when you arrive on campus. These are listed in section III of the checklist.
