Distance Education Forms

For SSC Faculty:

  • Instructor request to schedule an exam in the DEPC - Faculty now use SignUp Genius to schedule exams. Check your email for instructions.

  • Late/Make-Up Exam Form - use if a student needs to take a test before or after the regularly scheduled exam (do not add this to the DEPC Faculty Exam Schedule in Sign Up Genius)

Yes. Once the DE Coordinator emails approval, you must send the form to the Instructor.

Two weeks before exam


To submit forms, make a digital copy and email them to the instructor (for pre-approved forms) or the DE Coordinator, Michael Schnell, at (for approving proctors). An easy way to digitilize is simply to take a picture of the forms with a cell phone and attach it to the email. Some apps, such as Tiny Scan or Adobe Acrobat, can convert images to PDFs.
