SSS/SSS-STEM Application Forms
If you are interested in joining either the SSS or STEM program, please fill out the applications below. If you would prefer to fill out the application forms personally, the PDF download links are provided below. All application that are filled out manually can be e-mailed to j.wilsonbyrd@sscok.edu.
For DEPENDENT Students, completed Application and Income Verification forms are required for consideration. DEPENDENT students may submit a copy of previous year’s tax forms (showing Taxable Income) in lieu of the Income Verification form.
Applications are processed as soon as possible.
